Evaluation of hand hygiene compliance and associated factors with a radio-frequency-identification-based real-time continuous automated monitoring system

背景
手指衛生は医療関連感染症を予防するための主要な方策である。遵守不良を理解する上で 1 つの決定的に重要な点は、手指衛生を系統的にモニタリングするために用いる意味のあるマーカーが存在しないことである。
方法
本研究では、17 の個室病室を有する感染症病棟において、RFID ベースのリアルタイム連続的自動モニタリングシステムを用いて手指衛生遵守と関連因子の分析を行った。医療従事者を対象に、171 日間にわたりルーチンケアの実施中に追跡を行った。マルチレベル多変量ロジスティックモデルを用いてデータの解析を行った。主要転帰評価項目は、病室(区域外使用)への入室前および患者ケア区域に入る前の手指消毒とし、患者ケア区域は患者ベッド周囲の区域(区域内/ベッドサイド使用)と定義した。解析対象とした変数は、医療従事者の特徴および行動、患者、病室のレイアウト、医療従事者の行動経過の連鎖と持続時間などとした。
結果
患者ケア区域に入る際の最初の手指衛生機会を伴う行動経過計 4,629 件を選択し、このうちそれぞれ 763 件(16.5%)、285 件(6.1%)および 3,581 件(77.4%)が区域外使用、区域内/ベッドサイド使用、および不使用と関連した。手指衛生はケア提供者に依存している。医療従事者の行動経過の持続時間が短いほど、ベッドサイドの手指衛生が不良であった。1 ~ 2 名の別の医療従事者が病室内にいる場合は、ベッドサイドの手指衛生が改善した。
解釈
連続的モニタリングシステムを用いた分析によると、ベッドサイドにおける手指衛生の遵守は医療従事者の職種および個人的行動、医療従事者の人数、病室内で過ごす時間および(おそらく)ディスペンサーの設置場所に依存していた。食事トレーの配布は、手指消毒が行われないケースの潜在的な因子である。

監訳者コメント
手指衛生がし易い医療環境とそうでない医療環境、結果はおのずとしてし易い環境を医療者に提供することが必要であることを科学的に明確にした論文である。
