脳室ドレーン関連感染のサーベイランス:方法の提案とパイロット研究の結果

背景
脳室ドレーンの挿入は、一部の神経外科患者で不可欠であるが、髄膜炎・脳室炎のリスクを増加させる。よく知られたリスク因子はあるが、髄膜炎・脳室炎を発症する患者の割合は、定義の違いのために多少異なる。脳室ドレーンに関連した髄膜炎・脳室炎の定義について合意するため学際的作業グループを設立し、英国およびアイルランドの 4 施設でサーベイランスシステムを試験的に実施した。
方法
定義は、これまでに公表された定義と、臨床的および微生物学的基準に基づいて合意された。合意されたデータセットを開発し、脳室ドレーンの挿入後から、脳室ドレーンを抜去し微生物的原因を記録するまで、患者をモニタリングした。
結果
4 つの神経外科施設が参加し、各施設の患者 61 ~ 564 例を調査した。ドレーンの大多数は頭蓋内ドレーンであった。脳室ドレーン挿入のもっとも多い適用は頭蓋内出血であった。脳室ドレーンの 6% ~ 35%は、若手医師ではなく上級専門医に挿入された。髄膜炎・脳室炎を発症した患者の割合は 3% ~ 18%と異なり、1,000 脳室ドレーン・日あたり 4.8 ~ 12.7件であった。コアグラーゼ陰性ブドウ球菌が微生物学的原因でもっとも多くみられた。
結論
脳室ドレーン挿入患者の髄膜炎・脳室炎を in situ 検出するための定期的および持続的モニタリングは、物流の問題があり、ほとんどの施設で実施されていない。今回のパイロット研究が示唆することは、合意された定義を用いた全国的サーベイランスと、脳室ドレーンによる髄膜炎・脳室炎の施設間の比較を可能にする方法は、いずれも必要であり、実現可能であることである。これらのことから、今度は、感染を最少限に抑える質の改善プロセスの情報が得られるであろう。
