本研究では、2012 年から 2015 年にかけて、小児集中治療室における積極的感染制御およびサーベイランスの実施が、デバイス関連感染症の発生率に及ぼす影響を評価した。小児集中治療室には患児 1,378 例(4,632 患者日に相当)がおり、29 件のデバイス関連感染症が発生し、発生率は 2.1%、発生密度比は 1,000 患者日当たり 6.26 であった。中心ライン関連血流感染症、人工呼吸器関連肺炎、およびカテーテル関連尿路感染症の発生率は、それぞれ 1,000 カテーテル日当たり 7、1,000 人工呼吸器日当たり 1.17、1,000 カテーテル日当たり 0.24 であった。小児集中治療室におけるデバイス関連感染症の全発生率は低かったが、中心ライン関連血流感染症の発生率が相対的に高かった。

監訳者コメント
パキスタンの小児 ICU のサーベイランスであるが、小児集中治療では様々なデバイスを長期間使用することがあるため、国を問わず「患者の安全確保としての感染対策」は重要である。発展途上国でのデバイス関連の発生率は 2% ~ 27%と報告されており、本論文のパキスタンでの全体の発生率は報告より低いものであったが、中心静脈カテーテル関連感染の発生率が高いことは、カテーテルの挿入から維持管理、さらには輸液調整において、見直しが必要であろう。一方で多剤耐性菌の割合は、エンテロバクター、大腸菌、アシネトバクター、腸球菌において高率に認められることは、今後注意が必要である。
