Pharmacists’ impact on opportunistic infection prophylaxis in patients with HIV/AIDS

薬剤師は、ヒト免疫不全ウイルス／後天性免疫不全症候群患者のケアにおいて、抗レトロウイルス療法に関連する患者の転帰を改善することで、プラスの影響を与えることができる。本研究では、日和見感染症の予防薬の処方に薬剤師が介入することの影響を、介入の有無で比較した。スクリーニングされた患者 139 例のうち、42 例を前向き介入群に組み入れた。15 例に対して 27 回の介入が行われたが、そのうち 24 回（89％）で薬剤師による提案が受け入れられ、処方が変更された。後向きで調査した対照群と比較して、薬剤師の介入を加えることにより日和見感染症予防薬の処方が 58％から 93％に増加した（P ＜ 0.001）。

HIV/AIDS 患者のケアにおいて、薬剤師は様々な、そして重要な役割を持つ。抗ウイルス療法（ART）の薬剤選択、相互作用、アドヒアランスの確保などであるが、関与するのは ART 以外の薬剤にも及ぶ。これには日和見感染症の予防薬も含まれるが、本研究は、特に CD4 陽性リンパ球数の低い入院患者において、薬剤師による介入が日和見感染症予防薬の使用率および内服率を高めたことを示している。
