Clostridium difficile-associated clinical burden from lack of diagnostic testing in a Chinese tertiary hospital

クロストリジウム・ディフィシル（Clostridium difficile）感染症（CDI）は世界中の病院でよくみられる下痢の原因であるにもかかわらず、中国のほとんどの病院では検査も利用できず、管理ガイドラインもない。この 2 年間の前向き研究では、水様下痢患者 276 例のうち CDI の占める割合は 23.1％であった。CDI の検査ができないことで、患者の 26.4％では不適切な管理が行われ、隔離予防策がなされずに院内伝播のリスクが増加していた。さらに症候性の毒素産生性 C. difficile 保菌者を退院させることによって、市中伝播のリスクを増やすことにもつながっていた。最新のエビデンスに基づいた診療ガイドラインの更新と CDI の診断検査の採用を、中国の病院においても推奨したい。

監訳者コメント：
中国の病院における CDI の実態については、臨床的に疑われない、検査が利用できない環境である、といった理由から、不明な部分が多い。本研究は広東省の 1,800 床の教育病院で行われ、便培養で検出された C.difficile について、PCR を用いて同定とトキシン産生の決定を行った。下痢患者中、CDI の占める割合が高かったが、市中発症例が 35％を占めていたことも特徴であった。診断・管理の状況のみならず、今後、中国の他の地域からの疫学調査の報告が待たれる。
