Disinfection of gloves: feasible, but pay attention to the disinfectant/glove combination

背景
手指衛生の遵守は、患者 1 例のケア中に立て続けに手指消毒が必要になることで複雑化している。そのような状況では、手袋の消毒によりさらに良好なワークフローが促進され、遵守率が最適になると考えられる。
目的
5 つの異なる手指消毒液を 3 つの異なる手袋のタイプと組み合わせて、個々の効果を比較することにより手袋消毒の効果を解析した。
方法
調査は、DIN EN 1500:2013 にしたがって実施した。すべての組合せについて、以下の解析 10 回を実施した。（1）右／左手指の検査、1 回目および 5 回目の E. coli K12 NCTC 10538 による汚染後の消毒効果、（2）汚染後の回収率、（3）減少効果、（4）手袋の指先を浸漬した試料の培養、（5）漏れがないかのチェック。手袋を着用しない手指の消毒を追加のベンチマークとして実施した。
結果
すべての消毒薬／手袋の組合せの消毒効果は、手袋なしの場合より良好であった。8 つの組合せで、消毒効果は常に 5.0 log₁₀ 超であった。手袋および消毒薬のうちで有意差が認められた（それぞれP ＝ 0.0021、P ＝ 0.0023）。詳細を述べると、手袋では、Nitril Blue Eco-Plus の成績は Vasco Braun および Latex Med Comfort よりも有意に良好であった（それぞれ、P ＝ 0.0017、P ＝ 0.0493）。消毒薬では、Descoderm の成績は Promanum pure よりも有意に不良であった（P ＝ 0.043）。漏れのチェックでは、全サンプル中 Vasco Braun 手袋のみ漏れがなかった。手袋の消毒のしやすさについては重要な質的な差があった。
結論
様々な消毒薬／手袋の組合せの消毒効果は手袋なしの手指よりも大きかった。しかし、様々な消毒剤／手袋の組合せには消毒効果に関して重要な差が生じている。

監訳者コメント：
「手袋の消毒はダメ」何度も聞いてきたし、言ってきた言葉である。本研究はこの言葉に真っ向から挑んだものである。実際研究では相応の消毒効果はあるようだが、一方で消毒後の手袋の破損も見られている。消毒薬と手袋の相性もあるようである。「モノを大切にする」ことも大事ではあるし、「単回使用だから、単回使用なのだ」というのも、理不尽な気もする。本研究が明日からのプラクティスを変えるわけではないが、世間ではこのような研究が行われ、論文となり掲載されているという事実も知っておくと良いのではないだろうか。
