背景
クロストリジウム・ディフィシル(Clostridium difficile)感染症(CDI)の発生率と重症度は、近年上昇している。予測モデルは、感染症発症前にリスク患者を同定する上で有用となり得る。高リスク患者を早期に同定することは、抗菌薬管理プログラムや、高リスク患者で CDI を予防するためのその他のイニシアチブにとって有用となり得る。
目的
入院時に CDI の高リスクを有する患者を同定する予測モデルを作成すること。早期に同定するためのこのモデルを評価して、既に実施されている抗菌薬管理プログラムの効果を改善できるかどうかを明らかにした。
方法
ロジスティック回帰分析および受信者動作特性(ROC)曲線解析を用いて、入院患者の後向きコホートにおける入院時の CDI のリスクを予測する解析モデルを作成した。このモデルを前向きコホートで検証した。モデルによるリスク予測と既に実施されている抗菌薬管理プログラムとの一致度を評価した。
結果
本モデルは、後に検査陽性により CDI のリスクが高いとされた患者の 55%を入院時に同定した。抗菌薬使用に関する修正可能なリスク因子を有する高リスク患者 32 例当たり 1 例が、CDI について検査陽性であった。院内の抗菌薬管理プログラムのリスク基準を満たす前に、患者の半数(53%)が検査陽性であった。
結論
解析モデルにより、後に C. difficile 検査陽性を示した多くの患者を、入院時に前向きに同定できた。この早期同定のアプローチは、CDI 予防を改善するためにより早期の段階で感受性検査および抗菌薬療法変更を進める上で、抗菌薬管理プログラムにとって有用となる可能性がある。

CD 感染の予測モデルを作成し、その有用性を検討した論文である。ゴールは CD の予防である。実際にどの程度感染制御に寄与するのか、続報が楽しみである。
