イタリアの血液透析センターにおける C 型肝炎ウイルス（HCV）のアウトブレイクの調査から、患者 3 例が同じウイルス株に感染しており、同じ部屋において同じ時間帯に血液透析を受けていた慢性 HCV感染患者 1 例のウイルス株と同一であったことが示された。我々の観察研究の分析により、可能性のある伝播経路が多数特定されたが、今回の小規模クラスターにおいて HCV 拡散の経路は特定できなかった。このアウトブレイクでは、医療スタッフによる血液媒介感染症に対するスタンダードプリコーションの不履行が原因で、血液透析患者に対して HCV 院内伝播の機会が繰り返し生じていたことが確認された。

監訳者コメント：
透析ユニットで発生した C 型肝炎アウトブレイクの疫学調査の論文である。患者毎の手袋の交換や手指衛生、薬液調整エリアの管理など基本的な感染対策が遵守されていなかったなど複数の問題点が記載されていたが、マニュアルの整備や教育のシステムが十分に機能していなかったため、それらの問題点を改善する機会がなかったのかも知れない。
