Can the design of glove dispensing boxes influence glove contamination?

背景
臨床の場で手袋ボックスの細菌汚染を検討した研究はほとんどない。本観察研究の目的は、手袋 1 枚が縦に袖口の側から先に出てくる新たな手袋包装システムでは手袋および箱の開口部周辺表面の汚染が従来の手袋ボックスより低レベルであるかどうかを調査することであった。
方法
研究に参加する 7 施設に、縦型手袋ディスペンサーシステム（改良ボックス）と従来のボックスを供給した。手袋ボックスを開ける前に、開口部周辺の表面から微生物学的試料を採取し、表面汚染のベースラインレベルを確定した。いったん手袋ボックスを開けたら、各ボックスの最初の 1 組の手袋を生菌数検査用試料とした。その後、研究参加施設は 6 週間にわたり毎週訪問を受け、同様の微生物学的評価を実施した。
結果
改良ボックスの開口部付近表面は、従来のボックスに比べ、経時的に有意に汚染が少なく（P ＜ 0.001）、開口部周辺の汚染は平均 46.7％少なかった。全体として、改良ボックスからの手袋は、従来のボックスからの手袋に比べ有意に少ないコロニー形成単位を示した（P ＜ 0.001）。全施設を 6 週間全体にわたって比較したところ、改良ボックスからの手袋は細菌汚染が 88.9％少なかった。
結論
手袋ボックスデザインのこのようなシンプルな改善により、未使用手袋の汚染が減少する。そのような改良により、手袋を使用する場での微生物交差伝播のリスクを低下することができると考えられる。しかし、そのような利点が、厳格な手指衛生遵守と非無菌の単回使用手袋の適切な使用の替わりになることはない。

監訳者コメント：
そもそも問題は、汚染した手で手袋のような個人防護用具を取り出すところにある。そういう意味では、デザインの改善により開口部の汚染が減っても、取り出される手袋表面の汚染は避けられない。「個人防護用具を取り出す際には、手指衛生を行ってから取り出す」という教育を徹底することが重要である。
もちろんそれはそれとして、本論文で紹介されているような「モノ」側の改善が行われることも重要ではあるが。
