Accurate coding in sepsis: clinical significance and financial implications

敗血症は、世界的に、重大な医療問題であり死亡の主要な原因である。英国の院内死亡率の統計値および診療の報酬支払い額は、臨床コーディングデータをもとに計算されている。これらのデータの正確度は、コーディングの質に左右される。本研究の目的は、有意な菌血症を有する患者が敗血症とコードされているかどうかを調べ、ミスコードの経済的コストを試算することであった。1 か月間に有意な菌血症を有した患者 54例のうち、敗血症とコードされていたのはわずか 19％であった。このために不当に高い院内死亡率が算出されている可能性がある。さらにはこの結果、1 か月間のみで 21,000 ポンドの過少支払いが生じていた。

監訳者コメント：
英国の医療機関では ICD-10 を用いて、患者疾病のコーディングがなされ、診療報酬の支払いと死亡率の算出が行われている。今回の検討では，菌血症例の臨床情報の詳細を確認し、実際に敗血症・重症敗血症・敗血症性ショックであった例が正しく病名登録されていたかを調べたものである。有意な菌血症例 54 例のうち、敗血症の基準を満たしたのは 50 例であったが、このうち敗血症と登録されていたのは 10 例（19％）に過ぎなかった。つまり診療報酬はこの分過少に請求されており、また菌血症の死亡率も高く算出されていたことになる。同様の状況は、日本の保険病名登録でも認められると考える。
