Carbapenem-resistant Enterobacteriaceae on a cardiac surgery intensive care unit: successful measures for infection control

背景
カルバペネム耐性腸内細菌科細菌（CRE）は、集中治療室（ICU）での手術部位感染症（SSI）の原因である。本研究では、CRE 保菌を減少させるための介入および制御対策の影響、ならびに CRE アウトブレイク後の心臓外科病院 ICU の患者における感染率を評価することを目的とした。
方法
心臓外科手術後の成人の ICU における保菌患者または感染患者コホートの介入前および後の状態を評価する観察研究を 2013 年 4 月から 2014 年 12 月に実施した。通常のスクリーニング対策およびコホート看護とともに、ベッドサイドにアルコールゲルを用意し、洗い流さない 2％クロルヘキシジン含浸タオルを用いて毎日清拭し、1 日 3 回患者周辺の表面を消毒することで介入期間中の制御対策を強化した。
結果
CRE 保菌率（P ＜ 0.001）、中心ライン関連血流感染率（P ＜ 0.002）、SSI率（P ＜ 0.003）は介入後期間に有意に低下した。
結論
実施した対策は、心臓外科 ICU において CRE 保菌および感染の制御に有効であった。

監訳者コメント：
CRE の制御にクロルヘキシジンによる患者の清拭、環境消毒、手指消毒の 3 つの強化が有効であったことを報告している。どの方法もすでに多数の研究でその有用性が示されており、それらを組み合わせることで CRE のコントロールもできることが期待される。
