Qualitative study of views and experiences of performance management for healthcare-associated infections

背景
標準設定、モニタリング、およびインセンティブを用いた中央主導の成績管理方法は、医療制度における感染予防・制御（IPC）の顕著な特徴となっている。
目的
イングランドの病院における IPC に関連した規制および成績管理の見解と経験を明らかにすること。
方法
医療従事者および管理者との面談 139 件からなる 2 つの定性的データセットを分析した。成績管理および IPC に直接関連するデータを抽出した。データ解析には継続的比較法を用いた。
結果
参加者は、成績管理方法が特定の感染を中心にして行動を誘発したと回答した。説明責任を果たす組織構造を確立する利益が、感染率を減少させた経験的エビデンスの中で認められた。しかし、成績管理は全く無害なものとして経験されたわけではなく、1 つの領域において目標を設定することは、視野を狭くし、他の重要と考えられる課題を過小評価するリスクを伴うことが認められた。金融制裁は特に否定的にみられており、成績管理は、恐怖の文化をもたらし、学習を抑制し、専門職間の関係を崩壊させるリスクと関連していた。
結論
中央主導の成績管理が IPC におけるいくつかの重要な役割を担う場合があるが、IPC が妥当である場合を特定し、その限界を検討することはきわめて重要である、厳しい方法を持続することは、人間関係に影響を及ぼし、改善の努力を継続的に行うことへの抵抗を増す可能性があるが、すべての改善を現場のチームに任せることは欠陥のある戦略であるかもしれない。

成績管理に関するネガティブな影響は、もっともな結果である。評価することは大切だが、改善を目指してその先の対応をすることに現場は苦労している。そのことを再認識できた。
