Relationship between healthcare worker surface contacts, care type and hand hygiene: an observational study in a single-bed hospital ward

本研究では、手指衛生と、病室における医療従事者の表面への接触頻度との関係を定量化した。表面への接触と手指衛生を、英国の個室型の 1 病棟で 6 種類のケアについて記録した。表面への接触は、多くの場合ランダムでないパターンを示したが、患者への接触の前後での手指衛生はケアの種類に有意に依存している（P ＝ 0.001）。手指衛生遵守率は表面への接触回数と相関したが （95％信頼区間 1.1 ～ 5.8、P ＝ 0.002）、病室で過ごした時間とは相関しなかった。このことは、医療従事者が無意識に手指衛生に対する潜在的な必要性を感じていた可能性があることを示しており、この所見は、今後手指衛生教育プログラムに焦点を当てる上での支持と支援につながると考えられる。

監訳者コメント：
WHO手指衛生 5 つのタイミングでは、患者ゾーンに滞在している間は清潔無菌操作や体液曝露がなければ、環境表面に触れてもその前後に手指衛生を必要としていない。医療従事者による環境への接触と手指衛生の関係は非常に複雑であり、本研究結果なども踏まえて今後引き続き検討が続けられていくであろう。
