Sequence type 72 community-associated meticillin-resistant Staphylococcus aureus emerged as a predominant clone of nasal colonization in newly admitted patients

入院患者における市中感染型メチシリン耐性黄色ブドウ球菌（community-associated meticillin-resistant Staphylococcus aureus；CA-MRSA）の保菌に関する現在の知識は不完全である。韓国で新規に入院した患者から得た鼻腔定着 MRSA 分離株 637 株の遺伝型特性を検討した。配列型（ST）72が、2007 年から 2008 年、2009 年から 2010 年、2013 年から 2014 年の分離株のそれぞれ 52.1％、46.3％、52.8％を占めた。ST5 および ST239 など医療関連感染症の原因である古典的 MRSA クローンではなく、市中型である遺伝子型 ST72 の MRSA が新規に入院した患者において年齢や出身を問わず優勢株であった。医療関連 CA-MRSA 感染を予防するため、積極的な戦略が必要である。

監訳者コメント：
韓国で流行しているとされる ST72 に分類される市中感染型 MRSA の鼻腔保菌率の増加に関する報告である。ST72 は毒性が高いとされる USA300 と同様に SCCmec type IVを保有するが、PVL は陰性である点で USA300 とは異なる。ST72 は 2014 年に日本で報告されているが、バンコマイシンに低感受性であることが特徴であった。わが国ではまだ韓国と同様の保菌率の上昇などは報告されていないが、ST72 に限らず、わが国でも継続的なサーベイランスにより動向を注視する必要がある。
