Early impact of rotavirus vaccination in a large paediatric hospital in the UK

英国の大規模小児科病院において、市中感染および医療関連ロタウイルス胃腸炎に対するルーチンのロタウイルスワクチン接種の影響を、13 年間にわたり調査した。2002 年 7 月から 2015 年 6 月に入院した 0 ～ 15 歳の小児計 1,644 例が、ロタウイルス検査陽性であった。分割時系列分析により、ワクチン導入後（2013 年 7 月から 2015 年 6 月）、市中感染および医療関連ロタウイルス胃腸炎による入院は、それぞれ予想よりも 83％（95％信頼区間［CI］72 ～ 90％）および 83％（95％CI 66 ～ 92％）低いことが示された。ロタウイルスワクチン接種により、市中感染および医療関連ロタウイルス胃腸炎の両方の症例における院内ロタウイルス疾患の負荷が迅速に減少した。

監訳者コメント：
国内でも任意接種として受けることができるロタウイルスワクチン接種であるが（日本ではロタリックスが 2011 年 11 月に、ロタテックが 2012 年 7 月に発売）、定期接種化への移行が課題である。ワクチンで防ぎうる感染症の 1 つであるロタウイルス感染症については、医療関連感染予防も含めて接種率を高めるためには定期接種の対象に含めることが望まれる。
