Role of parents in the promotion of hand hygiene in the paediatric setting: a systematic literature review

背景
小児が入院する際、親は小児を保護する役割を病院と分け合い、小児に安全な医療（よい手指衛生習慣の実践を含む）を提供するために医療従事者との協力関係を築く必要がある。
目的
小児医療現場の手指衛生促進に対する親の参加についての科学的エビデンスをレビューすること。
方法
以下の用語を用いて、MEDLINE、EMBASE、Sci-ELO の各データベースを系統的に検索した：（‘hand hygiene’[MeSH] OR ‘hand hygiene’ OR ‘hand disinfection’[MeSH] OR hand disinf^* OR hand wash^* OR handwash^* OR hand antisep^*) AND (parent OR caregiver OR mother OR father OR family OR families OR relatives）。質の評価には Integrated Quality Criteria for Review of Multiple Study Designs のツールを用いた。
結果
文献調査により論文 1,645 報が得られ、研究 11 件が最終的な分析の選択基準を満たした。大半は観察研究であり、質問票または面接に基づくものであった。大部分の親は、手指衛生実施の適応に関する知識がほとんどなかったが、手指衛生を医療関連感染の予防に関係のある手段と認識していた。親が医療従事者に手指衛生の実施機会を逃さないよう促す意思にはばらつきがあり、全体としてかなり低かった。医療従事者に手指衛生を促すよう予め依頼されていた親は、医療従事者に手指衛生をより促しやすいと感じていた。
結論
この問題に関する文献は十分ではない。親による手指衛生の促進は、小児医療現場における患者安全を促進する可能性のある介入として調査することによりさらに検討する必要がある。

監訳者コメント：
手指衛生に限らず、医療への患者参画は良い影響をもたらすことが報告されている。特に小児領域は親の関与が原則となっており、患者・医療従事者双方に対して手指衛生の順守に与える親の影響は大きいだろう。
