Combined education and skin antisepsis intervention for persistently high blood-culture contamination rates in neonatal intensive care

血液培養の汚染（コンタミネーション）は、診断、入院期間、抗菌薬使用、薬剤および検査費用に関わってくる難しい課題である。新生児の血液培養において高率（3.8％）にコンタミネーションが生じたため、我々はこれまでの 70％イソプロパノールに代えて、2％グルコン酸クロルヘキシジン ＋ 70％イソプロパノールの無菌アプリケータを用いて採血前の皮膚消毒を行うとともに、スタッフ教育を併用した介入を推進し、成功した。介入前の 6 か月間に採取された新生児の血液培養では、364 検体中 14 検体（3.8％）が汚染されていたが、介入後の 6 か月間では 314 検体中 3 検体（0.96％）であり、有意な改善が認められた（Fisherの正確検定 P ＝ 0.0259）。皮膚関連の副作用は認められず、持続的な改善が得られた。

監訳者コメント：
新生児の血液培養は、皮膚の状態、採取場所、消毒の徹底、採取時の手技、アレルギーといった問題を抱え、汚染率も高くなりやすい。本検討ではグルコン酸クロルヘキシジンの濃度を 2％とし、採取に関するスタッフ教育も併せて行ったうえで、汚染率を有意に低下させることができた。これまで小児、特に新生児では血液培養時の消毒薬に関する検討が少なかったことに加え、クロルヘキシジンの濃度による効果の違いと安全性についても結論がついていなかったことから、今回の結果は貴重な情報といえる。
