Preventing ventriculostomy-related infections with antibiotic-impregnated drains in hospitals: a two-centre Dutch study

本観察コホート研究では、オランダの 2 病院において、脳室造瘻術関連感染症（VRI）に対する抗菌薬含浸脳室ドレーンの導入の効果を、通常のシリコン製脳室ドレーンと比較して、診療に他の変更を加えずに評価した。VRIの定義には、米国疾病対策センターの基準に加え、培養結果に基づく定義を用いた。プロペンシティ・スコア調整済み競合リスク生存分析により、抗菌薬含浸脳室ドレーンの導入は、交絡イベントと競合イベントの補正後も、通常診療においてVRIのリスクを有意に減少させず、また原因微生物の種類にも影響を及ぼさなかった。

監訳者コメント：
本研究は、通常のシリコン製ドレナージチューブに代わり、クリンダマイシン・リファンピシン含有のドレナージチューブを用いて脳室ドレナージを行い、留置後の髄膜炎・脳室炎の発生率や培養陽性に差があるかどうか調査したものである。本研究では減少は見られなかったが、今後も留置例は確実に存在し、かつ耐性菌感染症のリスクも増加するため、本テーマに関する動向には注目していく必要があろう。
