Compliance with hand hygiene: reference data from the national hand hygiene campaign in Germany

手指衛生は医療関連感染予防の重要な評価指標である。全国的に手指衛生を推進するために、2008 年 1 月、世界保健機関（WHO）の「Clean Care is Safer Care（清潔なケアはより安全なケア）」イニシアチブに基づき、ドイツのキャンペーン「Aktion Saubere Hände（手指衛生行動）」が開始された。本稿では、更新された観察ツールを用いて記録した手指衛生遵守に関する 1 年間の収集データから最初の結果を報告する。データは、109 の参加病院が 2014 年 1 月 1 日から 12 月 31 日に 576 病棟から収集し、提出を受けたものに基づいた。全体の遵守率の中央値は 73％、範囲は 55％（10 パーセンタイル値）～ 89％（90 パーセンタイル値）であった。結果は、成人の集中治療室（ICU）と成人以外の ICU で差はほとんどみられず、新生児用 ICU と小児の非 ICU は、成人の治療室より高い遵守率を維持していた。看護師の成績は医師より良好であり、手指衛生成績の総合的な率は、患者と接触する前より接触した後の方が有意に高かった。

監訳者コメント：
全体の遵守率が 73％ととてもよい結果、という印象であった。日本では、同様の報告としては、崎浜らの調査で処置後の遵守率が 32.7％となっていた J Hosp Med. 2016 Mar;11(3):199-205 http://www.ncbi.nlm.nih.gov/pubmed/26427035）。手指衛生遵守率は、ホーソン効果などもあり、観察の方法で大きく異なってくるが、それにしても、どのような職員教育を行っているのかが知りたい。
