Comparison of bacterial counts in expressed breast milk following standard or strict infection control regimens in neonatal intensive care units: compliance of mothers does matter

感染制御法 2 種（標準および厳格）のいずれかにより女性から得た搾母乳 1,466 検体中の細菌数を検討した。全体で、検体の 12％からグラム陰性菌が得られ、標準の方法（11.9％［788 個中 94 個］）と厳格な方法（12.1％［678 個中 82 個］）とで有意差は認められなかった（P ＝ 0.92）。家庭で搾乳した場合、汚染された検体が有意に多かった（標準の方法 家庭／病院 17.9％対 6.1％、厳格な方法 家庭／病院 19.6％対 3.4％、P ＜ 0.001）。搾母乳の細菌汚染は感染制御法と関連していなかったが、母乳を搾乳する場所と関連していた。母乳採取中の個人の衛生を改善する試みは価値が乏しいようである。採取および保管機器の良好な衛生が搾母乳の微生物学的な質の確保に最も重要な方法である可能性がある。

日本でも、NICU での感染症発生、伝播はしばしば問題となる。本論文は少々結果の解釈が難しいところがあるが、それにしても、母乳の管理や調乳プロセスのリスクアセスメントを行うことは非常に有用であることを再確認させてくれた。
