Norovirus outbreak management: how much cohorting is necessary?

ノロウイルスアウトブレイクを制御するために、曝露されたが症状のない患者を、曝露されていない患者および症状のある患者とは別に隔離することが広く推奨されている。隔離後、接触患者にノロウイルス感染症状が発現する頻度は系統的に検討されていない。我々は、2014 年 11 月から 2015 年 5 月の間に発生した 7 件のアウトブレイク（患者 57 例）の際の接触患者における典型的ノロウイルス症状の発現を後向きに検討した。典型的ノロウイルス症状を発現したのは接触患者 14 例中 1 例のみであり、このことは接触患者を隔離する現在の推奨に疑問を投げかけるものである。

監訳者コメント：
実際に発生したアウトブレイクをもとにした後向き研究である。サンプルサイズの問題、検査の問題、「曝露」の定義の問題など、数多くのリミテーションが考えられる。しかしこういった自施設での経験を学術的に記述し、リミテーションも明らかにしながら共有する姿勢を我々は見習うべきであろう。
