Reduction in Staphylococcus aureus bacteraemia rates in patients receiving haemodialysis following alteration of skin antisepsis procedures

本研究では、オーストラリアの Royal Darwin Hospital において 7 年の期間にわたり、血液透析コホートにおける黄色ブドウ球菌（Staphylococcus aureus）菌血症（SAB）の全症例について調査した。この期間の途中、動静脈瘻および中心静脈カテーテル（CVC）に対する消毒法が、0.5％クロルヘキシジン溶液から 2％クロルヘキシジン溶液へと変更された。登録データを用い、SAB エピソードの発生率を算出した。分割時系列の回帰分析を用い、SAB の経時的傾向を分析した。2％クロルヘキシジンへの変更後、SAB の発生率は平均で 68％低下しており、1患者・年あたり 0.111 件の SAB 症例が予防できたものと推定される。CVC に関連する SAB の発生率は試験期間を通して低いままであった。これらの結果は、動静脈瘻が造設された患者の皮膚の消毒における 2％クロルヘキシジンの使用を支持するものである。

監訳者コメント：
0.5％クロルヘキシジン溶液から 2％クロルヘキシジン溶液へと変更されただけで劇的な菌血症発生率の低下がみられている。個人的には 1％での影響をみてみたい。
