Outbreak of carbapenem-resistant Acinetobacter baumannii in the intensive care unit: a multi-level strategic management approach

複数の科にわたる集中治療室においてカルバペネム耐性アシネトバクター・バウマニー（Acinetobacter baumanni）のアウトブレイクが発生し、患者 10 例が罹患した。カルバペネム耐性アシネトバクター・バウマニーの伝播が認められてから数時間以内に、入手可能なデータを収集し、意思決定を行い、さらに集団隔離、一時的な入院の停止、職員の教育、および感染制御対策の強化などのすべての介入について情報伝達しモニタリングするための介入チームが結成された。カルバペネム耐性アシネトバクター・バウマニー保菌患者を集団隔離するためのエリアが規定され、別の看護チームと別の可動式機器一式が配置された。感染制御対策の強化の開始からわずか 4日後、新たな感染はもはや観察されなかった。

監訳者コメント：
インデックスケースは ICU 入室時に耐性菌保有がわかっていたため、接触予防策がとられていたが、その後、感染伝播が見られ、12 日後に 9 例目が確認されアウトブレイクと判断した事例の対応がまとめられた論文である。早期発見、早期対応の重要性が再確認される報告であった。
