背景
多剤耐性アシネトバクター・バウマニー(Acinetobacter baumannii)感染症による複数のアウトブレイクの調査により、病院内の無生物環境の汚染が、これらの多剤耐性株の拡散に関与している可能性が示された。
目的
アルジェリアの病院環境における、無生物表面でのカルバペネム耐性 A. baumannii の発生、ならびに起こり得る院内環境での拡散について、ヒトでの感染源である可能性を検討すること。
方法
A. baumannii 株を病院内環境から分離し、マトリックス支援レーザー脱離イオン化飛行時間型質量分析法(MALDI-TOF MS)により同定した。抗菌薬感受性を、ディスク拡散法および E テストを用いて評価した。カルバペネマーゼ活性を、修正 Hodge テスト、修正 Carba NP テスト、および EDTA テストなどの微生物学検査を用いて検出した。カルバペネム耐性の決定因子を、PCR 法および配列決定により検討した。クローン関連性を、multi-locus sequence typing(MLST)法を用いて判定した。
結果
計 67 株の A. baumannii 分離株を環境サンプル 868 個から回収し、MALDI-TOF MS により同定した。このうち、61 株がイミペネム耐性で最小発育阻止濃度が32 μg/mL 超であり、修正 Hodge テストおよび修正 Carba NP テストで陽性であった。さらに、カルバペネマーゼ活性は、32 株で EDTA により阻害された。PCR および配列決定により、29 株で blaOXA-23 遺伝子、32 株で blaNDM-1 遺伝子が認められた。MLST により、5 つの ST タイプ(ST19、ST2、ST85、ST98 および ST115)が認められた。
結論
本研究により、患者、医療従事者および訪問者を取り巻く病院環境内における無生物表面から回収されたカルバペネマーゼ産生 A. baumannii 株の拡散は、アルジェリアにおいて院内感染症の感染源である可能性が示された。

監訳者コメント:
OXA-51 と NDM-1 の遺伝子を有する株が多数の環境表面から検出され、それらはすべて ST85 であった。PFGE での相同性の確認があれば、同一の菌株であることがわかり、感染対策に生かすことができ、なおよかったのでは、と考えられた。
