緑膿菌（Pseudomonas aeruginosa）は、流し台や蛇口など、多くの様々な感染源から回収されている。緑膿菌は院内感染症の主要原因菌の 1 つであり、抗菌薬耐性の高まりによる病院内の環境リザーバは大きな懸念である。本稿で我々は、腎集中治療室における VIM カルバペネマーゼを発現する緑膿菌による感染症の 5 症例からなるクラスターについて報告する。本調査により、VIM カルバペネマーゼは汚染された蛇口に関連した手指を介して伝播することが示された。VIM カルバペネマーゼは、環境リザーバが存在する場合は、他の患者に交差感染する可能性がある。流し台と蛇口はよく考えてデザインし、完全な清掃および消毒を行うべきであり、また擦式アルコール製剤の使用を促進すべきである。

典型的な緑膿菌の伝播経路の 1 つについて調査した報告である。バイオフィルムを産生する緑膿菌の場合、ことに除染はやっかいである。時として蛇口や配管の交換が必要な場合もある。蛇口や排水溝の存在そのものがこうした施設内伝播する汚染菌発生源となるためエンジニアリングによる改善が求められる。
