Evaluation of patient-held carbapenemase-producing Enterobacteriaceae (CPE) alert card

英国公衆衛生庁は、カルバペネマーゼ産生腸内細菌科細菌（CPE）保菌者にカードの携帯を推奨している。この警告カードをCPE 陽性入院患者104 例に渡し、6 か月目に追跡調査を行った。死亡例を除き、回答率は 39％であった。16 例（46％）の患者はカードを受け取ったことを覚えており、うち 13 例（81％）は今なおカードを保持していて、7 例（64％）がカードを実際に使用したことがあると回答した。これは患者携帯型薬剤耐性菌警告カードについて英国で評価した初めての報告である。本研究で、CPE 警告カードは携帯されれば有用なコミュニケーションツールになり得ることが判明した。有効性の評価と保持率の改善にはさらなる研究が求められる。

監訳者コメント：
これまで心不全や酸素投与を受けている患者に携帯型のカードを持ってもらい、受診時などに提示することによって適切な管理をすみやかに行う試みがなされてきた。薬剤耐性菌での試みはあまり例がないが、前 2 者とは異なる意味があることも承知しておきたい。すなわち治療のみならず感染対策上の目的である。有用性と重要性は理解できるが、倫理上の配慮と、患者不利益にならないような運用をよく検討する必要がある。
