カルバペネマーゼ産生腸内細菌科細菌(carbapenemase-producing Enterobacteriaceae;CPE)に関する欧州の疫学的サーベイランスを強化する多大な努力にもかかわらず、いくつかの国からの情報は依然として乏しい。ポルトガルにおける CPE の疫学に取り組むために、我々は、2012 年に、3 次病院の微生物検査室で同定される CPE 培養を行った成人の後向きコホート研究を実施している。25 の病棟または集中治療室から 60 例の患者を特定した。我々の知る限りでは、これがポルトガルにおける CPE 臨床データの初の報告である。報告によると、病院全体での CPE の拡散が示され、これまでに記述されていない進展しつつある疫学的状況について我々に注意喚起している。

監訳者コメント
これまで、ポルトガルでのCPEの検出状況は不明であり、本論文が初めての後ろ向き調査によるCPEに関する疫学的報告である。ECDCのレポート1)においては、イタリアがendemic状況であるのに対して、ポルトガルでのCPEの発生状況は散発的ではあるが、疫学的状況は不明となっていた。日本においてもCPEはすでにいくつかの病院でアウトブレイク事例が報告されており、適切な抗菌薬治療ができないこと、腸管内に保菌すること、菌種を越えて耐性遺伝子がプラスミドを介して伝播することなどにより、拡大してしまうとその感染制御は困難であり、さらに感染症発症した場合の予後も良くないことが報告されている。CPE耐性菌の迅速な検出ができる体制と発生状況を監視できるサーベイランス体制を日本でも確立する必要がある。今年日本で開かれる伊勢志摩サミットにおいても「薬剤耐性菌」対策は大きな議題のひとつとなることを付け加えておく2)
