First outbreak of linezolid-resistant vancomycin-resistant Enterococcus faecium in an Irish hospital, February to September 2014

アイルランドの 3 次紹介病院の肝疾患病棟で、2014 年 2 月から 9 月にリネゾリド耐性バンコマイシン耐性エンテロコッカス・フェシウム（Enterococcus faecium）のアウトブレイクが発生した。リネゾリド耐性バンコマイシン耐性 E. faecium が患者 15 例から分離され、パルスフィールド・ゲル電気泳動により単一クローンの拡散であることが確認された。本稿は、アイルランドにおけるリネゾリド耐性バンコマイシン耐性腸球菌によるアウトブレイクの初の報告である。

アイルランドでは、E. faecium 菌血症における VRE の比率が、2013 年で 42.7％と他の EU 諸国の 8.9％に比較して極めて高く、日常的に VRE が分離されているものの、リネゾリド（LZD）耐性 VRE は 2008 年に単一例での報告のみで、集団発生は本事例が初めてである。このアウトブレイクの原因として、VRE 陽性患者に VRE スクリーニングの培養検査が繰り返し実施されていたが、同時に感受性検査が実施されていなかったことが LZD 耐性 VRE の早期検出を遅らせた原因と推測されている。日本での 5 類感染症「VRE 感染症」はまれであり、LZD の使用頻度も当然低い。しかし、MRSA 感染症治療において LZD は重要な治療薬であり、その使用頻度も高い。MRSA および VRE の感染症治療薬である LZD の後発品が、2015 年 6 月より販売されたが、「後発品は、VRE 感染症のみに適応をもつ」のであり、MRSA への適応はない。しかしながら、適応外使用や長期使用など不適切使用が実施された場合、本論文のような LZD 耐性の VRE の出現が危惧される。すでに LZD 耐性の MRSA は日本でも報告されており、LZD の適正使用が望まれる。
