Evaluation of excreta management in a large French multi-hospital institution

背景
排泄物は、基質特異性拡張型β-ラクタマーゼ（ESBL）産生株を含む多剤耐性腸内細菌科細菌の主要な感染源である。Assistance Publique － Hôpitaux de Paris（AP-HP）で ESBL 産生腸内細菌科細菌の保菌率が上昇したため、排泄物廃棄に用いる器具および実践状況に関して評価が必要となった。
目的
排泄物を扱う器具の使用状況を評価すること、および排泄物の廃棄に関する医療従事者の実践状況を調査すること。
方法
2012 年に横断研究を実施した。
結果
AP-HP の 28 病院の 536 部門（急性期病院 342 部門、リハビリテーション・長期ケア病院 194 部門）の評価を行った。調査日に在院していた患者のうち、5,697 例（43％）がおむつを使用しており、1,767 例（13％）がベッドパンを使用していた。病床の 61％には共用トイレが設置され、トイレの 43％にはハンドスプレー（糞便物質で環境の汚染を起こしやすい器具）が設置されていた。各部門の 68％にはベッドパンのウォッシャーディスインフェクターが設置されていた。ベッドパンのウォッシャーディスインフェクターのうち、擦式アルコール製剤が利用可能な病室内に設置されていたのは 52％のみであった。各部門の 71％ではベッドパンの洗浄を消毒に先立って実施しており、また半数以上は患者の浴室で行っていた（62％）。質問を受けた医療従事者のうち、排泄物の廃棄に関する教育プログラムに従っていると回答したのは 9％のみであった。
結論
本調査により、多剤耐性腸内細菌科細菌の制御や擦式アルコール製剤による手指衛生推進に関して、排泄物の管理は懸念事項であるものの、なおざりにされていることが示された。

監訳者コメント：
患者・医療従事者の両者において、糞便中の微生物が排泄行為に関連して手指や衣服、トイレなど周囲の環境を汚染し、医療関連感染の原因になることは、極めて重要な伝播経路であるにもかかわらず、それほど大きく意識されていない。事例の発生がないときにはなおさらであるが、日常的であることが、かえってなおざりになってしまう大きな要因であるかもしれない。本検討の結果を踏まえ、改めて見直し、対策の遵守と教育を考え直す機会にしたい。
