心臓手術後の人工呼吸器の使用により人工呼吸器関連肺炎(VAP)リスクが上昇することから、術前の 0.2%クロルヘキシジンが術後 VAP に及ぼす影響を調査するための前向きランダム化対照試験を実施した。心臓手術が予定されている患者 94 例をクロルヘキシジン群(47 例)または対照(生理食塩液)群(47 例)にランダムに割り付けた。患者は手術前日、毎食後 30 分と就寝時の歯磨き後 5 分に 0.2%クロルヘキシジンまたは生理食塩液で 3 回うがいをした。VAP 発生率はクロルヘキシジン群 8.5%、対照群 23.4%であった。術前のクロルヘキシジン洗口液により、術後 VAP の発生が有意に減少した。

監訳者コメント:
VAP は心臓手術後の重大な合併症であり、その予防は極めて重要である。心臓手術後 VAP の発生率は 2.3%から 45.9%と報告により大きく異なっている。これまでに挿管後 1 ~ 4 回/日程度の「クロルヘキシジングルコン酸塩(CHG)」による口腔ケアが実施され、VAP 発生率を減少させることが報告されている。本論文は心臓手術の前日から CHG のうがいを実施することで、術後の VAP が減少できたとの報告である。挿管 4 日以内に発生する早期 VAP は、口腔や咽頭の常在菌叢が主な原因となるため、挿管前に口腔内の細菌数を減らしておくという発想は理にかなっているかもしれない。本論文は症例数も少ないので、結論を出すにはさらなる前向きの大規模研究が必要である。なお、本論文で使用されている CHG は、日本において「口腔等粘膜への適用は禁忌」である。
