国際的紛争での負傷者はメディアに注目され、市民、政府、および非政府の組織の同情を集める。専門医による再建術や 3 次医療サービスを提供する先進国の病院が、複雑な外科的処置やリハビリテーションのために海外の紛争地から民間人患者の受け入れを要請されることはまれではない。こうした患者がもたらす感染予防・管理のリスクにかかわる懸念や、適用される予防策には良好なエビデンス基盤が不足していることから、ゼロリスクの考え方を受容する予防原則に従うことが多くなる。本稿の目的は、海外の紛争地からの負傷者を治療する際に必要であると考えられる感染管理上の留意事項を示すことであり、その一部は著者らの経験に基づくものである。現在、これらの患者の管理方法に関して公表されているエビデンスおよび全国的なコンセンサスは不足している。予防原則には、従来のリスク管理へ移行するためのエビデンスや知識の継続的な探索が求められる。個々の多数の病院が、このような少数の患者の治療経験を蓄積することによってのみ、これらのエビデンスや知識が浸透していくものと考えられる。
