人工呼吸器装着・気管切開患者に使用する蛇管のオゾンおよび超音波による消毒

背景
傷のある皮膚や粘膜に接触する医療器具は、セミクリティカル器具に分類される。あらゆる侵襲的処置の中で最も重大なリスクは病院感染の原因となる病原性微生物をもたらしてしまうことであるため、これらの器具には最低でも高レベル消毒が必要である。
目的
感熱性のセミクリティカル器具に対するオゾンガスおよび超音波の消毒能を評価すること。
方法
集中治療室の人工呼吸器装着・気管切開患者に使用した蛇管を入手した。酵素洗浄剤で 15 分間洗浄後に、以下の種々の消毒法を評価した。A 群(0.2%過酢酸)、B 群(超音波 60 分間)、C 群(濃度 33 mg/L のオゾンガス 15 分間処理)、D 群(超音波 30 分間、オゾン 15 分間)、E 群(超音波 60 分間、オゾン 15 分間)。
結果
60 分間の超音波処理による微生物汚染レベルの低下は 4 log10 であったが、オゾン単独やその他の 2 種類(超音波とオゾン)の併用および過酢酸では微生物汚染レベルが 5 log10 低下した。
結論
処理時間、使用の容易さ、有効性、および費用を考慮すると、オゾンは最も有用な方法であった。セミクリティカル器具の消毒にオゾンガスを使用することは技術的に可能であり、極めて有望であることが示された。

人工呼吸器回路に使用する蛇管の消毒におけるオゾンの有用性について検討した論文である。オゾンの有効性を十分に発揮するためには有機物の除去が必須であり、前処理として酵素洗浄剤による洗浄を行うことが重要である。またオゾンの使用にあたっては、吸入防止や適正濃度の管理などの注意が必要である。超音波処理だけでも相応の菌減少効果が得られるのは興味深い。
