集中治療室環境における医療用抗菌手袋の有効性の評価
Evaluation of the efficacy of antibacterial medical gloves in the ICU setting

背景
医療用手袋の不適切な使用が、微生物の伝播を促進する恐れがある。新たな戦略によって、患者や表面の汚染リスクを上昇させることなく、医療用手袋の安全性を強化することが可能であると考えられる。
目的
集中治療室環境における一般的な患者ケア後の手袋の汚染減少に関する有効性について、外表面をポリヘキサメチレンビグアナイド塩酸塩(PHMB)でコーティングした合成抗菌性ニトリル製医療用手袋と、同一タイプの非抗菌性医療用手袋を比較すること。
方法
集中治療室の職員が、患者のケア実施時に標準的な手袋または抗菌性手袋のいずれかを装着した。4 種類の臨床行動後に、直ちに手袋上の細菌数を半定量的に測定した。
結果
対照手袋と抗菌性手袋の平均細菌数(コロニー形成単位[cfu])には有意差が認められた(手袋インプリントあたりの平均値 ± 標準偏差、60 ± 23 対 16 ± 23 cfu、P < 0.001)。4 種類の臨床行動のうち 3 種類では、抗菌性手袋の細菌汚染は対照手袋と比較して有意に少なかった(静脈内輸液処置 P = 0.011、口腔洗浄 P < 0.001、および理学療法 P < 0.001[日本語版監訳者注:原著 Table 1 には P 値はそれぞれ 0.002、0.011、および 0.002 と記載されている])。抗菌性手袋では、対照手袋と比較してリネン類の交換後の細菌汚染が少なかったが、その差は有意ではなかった(P = 0.311)。
結論
本研究で評価した臨床行動では、医療用抗菌性手袋を使用することによって、一般的な患者ケア後の細菌汚染陽性率は 57%減少した(P < 0.01)。医療用抗菌性手袋の使用は、集中治療室環境における交差汚染の減少を促進すると考えられる。

監訳者コメント:
処置内容によっては有意差がでないという結果を導き出している。結果はいずれも抗菌性手袋の細菌汚染が少ないというものであり,その臨床的な影響について具体的に評価を進めていく必要があろう。
