Bordetella pertussis infection in paediatric healthcare workers

近年、成人に百日咳の発生が増加しているため、医療従事者は乳児へ伝播をもたらすリスク群であると考えられている。ブラジルの 3 次病院の小児科の医療従事者を対象として、百日咳菌に最近感染した割合を評価した。血清中の抗百日咳毒素 IgG 抗体を ELISA で測定した。評価対象の医療従事者 388 名のうち 6.4％で最近の感染が血清学的に示唆された。レジデント（オッズ比［OR］4.15、95％信頼区間［CI］1.42 ～ 12.14、P ＝ 0.009）、および労働時間が 40 時間を超える医療従事者（OR 3.29、95％CI 1.17 ～ 9.26、P ＝ 0.024）は、百日咳菌感染のリスクが高かった。

監訳者コメント：
日本でも、思春期以降の年齢層の防御抗体価の保有率が低いことから、2007 年には複数の大学でのアウトブレイクが発生した。大人向けの百日咳ワクチン（三種混合ワクチン）は認可されていないため、いくつかの医療機関では、医療従事者に三種混合ワクチン（DTaP）を半量接種するなどの対策がとられている。本論文の結果は、我々にとっても無関係ではないと思われた。
