Nationwide study on the use of intravascular catheters in internal medicine departments

背景
集中治療室（ICU）における血管内留置カテーテルの使用については、近年、十分な評価が行われている。しかし、これらのデバイスの多くは ICU 以外の患者、特に内科部門で留置されており、ケアの質に関するデータは少ない。
目的
スペインの内科部門における血管内留置カテーテルの使用および管理について評価すること。
方法
2013 年 6 月のある 1 日に、種々の規模の病院の 47 の内科部門の全成人入院患者を対象として点有病率調査を実施した。各施設で担当者を指名し、患者の評価およびデータ収集を行った。
結果
調査日に入院中の成人患者 2,080 例中 1,703 例（81.9％）が 1 本以上の血管内留置カテーテルを使用していた（このうち 95.4％は末梢カテーテル）。92 本（5.0％）のカテーテル挿入部位に感染が認められ、患者 87 例（5.2％）に敗血症の症状がみられたが、カテーテル関連と判定された症例は 1 件のみであった。施設担当者は、留置カテーテルの 19％はすでに不要であると判定した。カテーテルの必要性に関する連日の記録は 40.6％のみの症例で利用されていた。
結論
本研究から、スペインの内科部門におけるカテーテルの使用とケアについては、明らかに改善の余地があることが示された。ICU で適用されているものと同様の戦略を、内科部門でも実施するべきである。

監訳者コメント：
集中治療室などではなく、一般内科病棟における血管内留置カテーテルの使用状況に関するデータは少ない。本研究は点有病率調査という調査施設における負担が少ない手法を用いて行った全国調査である。日本でもこのような手法を用いて様々な感染防止策の現状を調査することが可能であろう。
