背景
開心術および冠動脈バイパス移植術後の手術部位感染(SSI)のリスク因子の相違については、十分に説明されていない。
目的
開心術および冠動脈バイパス移植術後の SSI のリスク因子を特定し、比較すること。
方法
院内感染対策サーベイランス(JANIS)システムに 2008 年から 2010 年に提出された開心術および冠動脈バイパス移植術の SSI サーベイランスデータの解析を実施した。単変量モデル解析、次いで多変量ロジスティック回帰分析により、SSI に関連する因子を解析した。非 2 値変数については、まず初めに最適なカテゴリーを特定した。
結果
SSI 累積発生率は開心術 2.6%(151/5,895)、冠動脈バイパス移植術 4.1%(160/3,884)であった。両手術群ともに、手術時間および米国麻酔学会(ASA)スコア高値は各モデルの SSI リスクの有意な予測因子であった。創分類は、開心術後の SSI と独立した関連が認められたが、冠動脈バイパス移植術では関連はみられなかった。インプラント、複数回の手術、および緊急手術は開心術後の SSI を予測したが、冠動脈バイパス移植術ではこれらの因子はいずれも SSI を予測しなかった。
結論
2 種類の心臓手術の間には、SSI リスクの予測に関する著明な相違が認められた。現行のサーベイランスシステムから入手可能な変数を組み入れることによって、開心術のリスク分類を改善できると考えられる。SSI の有意な予測因子であることが示されたのは、評価した変数のうち 2 つのみであったことから、追加的な変数を収集することによって冠動脈バイパス移植術のリスク指標分類を強化することができると考えられる。
