医療環境におけるアデノシン三リン酸(ATP)を用いた清掃モニタリング:環境内 ATP の減少速度

背景
表面消毒薬を適切に使用するためには、清掃職員の清掃遵守を確実なものとすることが不可欠である。アデノシン三リン酸(ATP)検査は、清掃遵守のモニタリング方法の 1 つとして推奨されているが、環境表面上の ATP の安定性についてはほとんど知られていない。
目的
医療環境の清掃・消毒の効果的な評価方法として、ATP の安定性が十分な期間にわたって持続するかどうかを明らかにするために、各種由来の ATP の安定性を評価すること。
方法
精製 ATP、ATS-T(血液含有試験用汚染物質)由来 ATP、および測定部位あたり 107 コロニー形成単位の微生物(緑膿菌[Pseudomonas aeruginosa]、エンテロコッカス・フェカーリス[Enterococcus faecalis]、カンジダ・アルビカンス[Candida albicans])由来 ATP の 29 日間の安定性を、懸濁液中および表面上の乾燥状態で評価した。乾燥させた表面に洗浄剤または消毒薬を噴霧して、拭き取りは行わずに、微生物の生存および ATP の安定性に及ぼす効果を評価した。
結果
緑膿菌、E. faecalis、および C. albicans を表面で乾燥させた場合は、29 日目には ATP は初期レベルの 65% ~ 96%が残存していたが、生菌数は減少するか、検出されなかった。表面で乾燥させた ATS-T は、4 日目には初期 ATP レベルの 100%、29 日目は 3%が残存していた。ATP シグナルの最も著明な減少は懸濁液中で認められた。精製 ATP は懸濁液中でも表面上の乾燥状態でも 29 日間にわたって安定していた。
結論
有機物質および微生物(死菌または生菌)由来の残存 ATP は、表面上の乾燥状態では安定していた。清掃および消毒を行わない場合は、相対発光量のシグナルの急速な減少は認められないため、ATP は清掃モニタリングの良好なマーカーとなる。

監訳者コメント:
近年、ATP が清掃の質評価のために使用されることが増えている。しかし、ATP が一般環境中でどのくらい安定なのかについてのデータがなかった。本研究では ATP は約 1 か月間、環境中で安定であることを報告している。掃除しなくても放っておいたら勝手に減ってくれる、わけではないようだ。
