Mandatory public reporting of healthcare-associated infections in developed countries: how can developing countries follow?

医療環境内での、または医療環境から市中への薬物耐性病原体の伝播増大がもたらす脅威は、極めて現実的であり憂慮すべきものである。先進国ではこれらの脅威を阻止するために強力な対策を講じているが、発展途上国に対して何らかの是正措置を取らせるような圧力はほとんどなかった。インドやその他の多くの発展途上国の病院で、医療関連感染率が警戒を要する場合の報告が義務化されていれば、利害関係者（医療従事者、議員、行政官、および病院における方針立案者など）によるこの問題の認識および対応の支援につながったと考えられる。この報告の義務化によって、医療における長年軽視されていた分野に質管理がもたらされ、インドには実質的に存在しない患者の権利向上が可能になると考えられる。医療機関は、医療関連感染予防の欠落に対する「ゼロ・トレランス」の強化に注力すべきである。世論の圧力によってインド政府がこの問題を認識し、より多くの資金をリソースおよびインフラの改善に割り当てることになると考えられ、その結果、平均的なインド国民に提供される医療水準が大幅に向上し得る。多くの困難が伴うものの、医療関連感染に関する全国的な公的ベンチマークを設定することは、インドなどの発展途上国におけるこの脅威への取り組みに大きな役割を果たすと考えられる、優れた目標である。
