この 20 年間で、世界規模の貿易や人の移動が急増している。このことは、抗菌薬耐性遺伝子の進化および動向に深刻な影響を及ぼした。世界中の人々が新興経済国で発生した耐性菌にますます曝露されている。抗菌薬耐性の領域におけるこの 20 年間の最も重要な出来事は、CTX-M 型基質特異性拡張型 β-ラクタマーゼ（ESBL）の出現と拡散であろう。アジア諸国では腸内細菌科細菌の ESBL 産生率が極めて高かったためにカルバペネム系抗菌薬が大量に使用されることになり、プラスミドを介するカルバペネム耐性が出現した。本稿では、多剤耐性グラム陰性菌の出現と拡散について概説し、3 種類の特定のカルバペネマーゼ、すなわちイミペネム・カルバペネマーゼ、肺炎桿菌（Klebsiella pneumoniae）カルバペネマーゼ（KPC）、およびニューデリー・メタロ-β-ラクタマーゼに焦点を当てるとともに、抗菌薬使用制限の重要性を明らかにする。
