Pseudomonas aeruginosa in hospital water systems: biofilms, guidelines, and practicalities

緑膿菌（Pseudomonas aeruginosa）は、日和見病原体として振る舞い、抵抗力の下がった患者に保菌・感染を引き起こす微生物の 1 つである。院内の水は緑膿菌の伝播源となることが知られており、給水システムの汚染によるアウトブレイクも報告されている。2011/12 年の北アイルランドの子供病棟での発生もその一例であるが、新生児 4 人の死亡を直接的な契機として、英国保健省は指針（Health Technical Memorandum 04-01 補遺）を作成し、国民保健サービス（NHS）の管理者を対象に、特別ケアユニット（augmented care unit）での緑膿菌の対処法に関する勧告を行った。この指針は、現時点での専門家の意見のほか、限られてはいるが、科学的なエビデンスに基づいている。Public Health England は、実験室環境で再現・制御を行う給水テスト装置を製作し、給水システムの緑膿菌汚染のさらなる研究と、細菌の易定着部位の特定を行うこととした。これらの研究によりエビデンス基盤が強化され、さらには指針の改訂に寄与することが期待される。

院内環境、それも水に関する緑膿菌の汚染は深刻な問題であり、いったん発生した場合、その制御は非常に困難である。各施設での個別、独自の対応を迫られることも多い。その早期発見と具体的な解決法について、感染対策担当者は普段から十分念頭に置いておく必要があるが、本レビューはその点で有用であり、さらに詳しい情報を入手するにも役立つと思われる。
