Antibiotic stewardship programmes and the surgeon’s role

不適切な抗菌薬使用は高頻度にみられ、特に外科部門で多い。このような使用による不要な代償は、患者にとって望ましくないアウトカム、および耐性細菌の出現・拡散である。抗菌薬管理プログラムの目的は、慎重な抗菌薬処方を推進することによって抗菌薬耐性の拡散を抑制することである。一般的に、このようなプログラムが目指していることは、抗菌薬の不適切な使用の制御、薬剤選択・用量・投与経路・投与期間の最適化、臨床的治癒や感染症予防の最大化、および望ましくない影響や過剰コストの抑制である。本稿では、抗菌薬の不適切な使用の影響について考察するとともに、これに対処する方法としては抗菌薬管理が最善であろうと著者が考える理由を概説する。外科医が抗菌薬管理プログラムに関与することが、その成功のためには極めて重要である。
