How infection control teams can assess their own performance and enhance their prestige using activity and outcome indicators for public reporting

フランスでは 1970 年代後半に感染制御担当者が登場した。フランスの保健当局は 1995 年、感染制御チームの概念を公式に導入し、1999 年 11 月に義務化した。2000 年には、各病院に感染制御に関する機密の年次報告が義務づけられた。消費者団体の圧力により、保健省は 2004 年に当局への報告用の感染制御成績指標を導入し、その最初のものは ICALIN として知られる。感染制御に関する年次報告は病院全体の報告として意図されたものであったが、実際は感染制御チームの報告と見なされることが多かったため、感染制御チームは感染制御指標に基づく所属病院の成績に対して責任を有するようになり始めた。いくつかの感染制御チームは、自らの活動、とりわけカウンセリングは、その分量と範囲が報告に反映できていないと考えた。しかし、チームの多くは、自らの業務が少なくとも注目の対象となり、有用であることが認められたため、当局への報告には有益性があると認識した。こうして、フランスでは感染制御成績を評価するための指標の使用は、感染制御チームを支援するものとなった。指標を継続させるためには、定期的な改良が必要である。フランスで病院衛生の専門家の成績と信頼を高めるためには、彼らのコア・コンピタンスのさらなる向上への取り組みが必要である。感染制御チームは所属病院における院内感染の責任を担うべきか？「ノー」という答えは奇妙に思われるであろうが、無条件の「イエス」もまた同様であろう。

監訳者コメント：
諸外国では医療関連感染に関する様々な指標を公開する施策が積極的に行われている。このような指標の公開には賛否両論があるが、フランスでは少なくとも感染制御領域においては一定の成功を収めているようだ。今後は日本でも同様の施策が導入されると考えられる。一歩先を行く ICT はぜひ目を通しておいてはいかがだろうか。
