Rapid bedside tests for diagnosis, management, and prevention of nosocomial influenza

インフルエンザウイルスは他の呼吸器ウイルスと同様に、ナーシングホームや一般病棟、救急部門で深刻な院内流行を引き起こす。患者、医療従事者、訪問者ともに院内発生インフルエンザの感染源になる可能性がある。インフルエンザの迅速診断検査は、感度は限られているものの実用的な価値はあると考えられ、これらの検査によって拡大予防策の早期導入や、症例に対する特異的な抗ウイルス療法の早期実施が可能になる。しかしながらこれらの検査法ではオセルタミビル耐性を検出できず、感受性検査は専門的な検査機関でしか実施されていない。耐性はまれではあるが、特に幼い子供や免疫不全者では治療中に出現することもある。免疫不全者では、耐性インフルエンザウイルスの排出が数週間にわたって持続することがあり、さらに患者間での院内伝播の散発例が報告されている。ベッドサイドの迅速診断検査には限界があるため、単独の診断ツールとしての使用には適していない。しかしこの検査は迅速、簡便で、かつ費用対効果にも優れており、その限界によって院内発生インフルエンザの検出およびその後の管理のための使用が妨げられるものではない。検査法の近年の進歩は有望なものであり、感度および特異度の改善と、抗ウイルス薬の感受性のスクリーニングが可能になると期待されている。

監訳者コメント：
迅速抗原検査は本邦ではインフルエンザの診断に多用されており、院内でインフルエンザが発生した場合の感染対策の介入、特に早期の封じ込めにも寄与している。しかしながら検査感度や検査前確率、キット間の違いに関する理解が十分とは言い難い。キットの改良・新規導入が進む中、それらの能力の評価のみならず、状況ごとの使用の妥当性をその都度、十分確認するべきであろう。
