Epidemiology of meticillin-resistant Staphylococcus aureus bloodstream infections in Alberta, Canada

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）血流感染症（BSI）に関する大半の研究は、単一または少数の病院からの便宜的標本に基づく。2011 年 4 月から 2013 年 3 月に、カナダ・アルバータ州の全病院で MRSA BSI と診断された症例を前向きに収集した。分離株の spa タイピングを行った。MRSA BSI 症例は合計 299 例であり、地域住民 100,000 名あたり 3.95 例に相当した。市中獲得型 BSI 症例が 66.9％、病院獲得型症例が 33.1％を占めた。症例は 3 次病院（36.4％）および都市部の大規模病院（34.3％）で多く認められたが、地域病院および地方病院でも多かった。小児病院では症例は極めて少なかった（3.0％）。CMRSA 10（USA 300、40.2％）と CMRSA 2（USA 100/800、38.0％）の 2 種類のクローンが優勢であった。

監訳者コメント：
本調査では、これまで一般に市中型と考えられてきた CMRSA（USA300）が市中感染の 55％、医療関連感染の 33.8％、院内感染の 27.6％を示し、院内型が主流と考えられてきた CMRSA2（USA100/USA800）が、それぞれ、27.5％、35.2％、53.3％を示しており、タイピングだけでは由来が想定できなくなってきていることを暗示していた。
