Is surgical site scrubbing before painting of value? Review and meta-analysis of clinical studies

背景
手術部位感染症は重大な手術合併症である。消毒薬擦り込み（painting）前の手術部位スクラブの意義については異論がある。
目的
術前の消毒薬擦り込み＋その前の手術部位スクラブと、消毒薬擦り込み単独による手術部位感染症の予防を比較した臨床研究のメタアナリシスを実施すること。
方法
PubMed、ScienceDirect、および Cochrane データベースを用いて、術前の消毒薬擦り込み＋その前の手術部位スクラブと消毒薬擦り込み単独とを比較し、かつ評価項目として手術部位感染症、皮膚保菌、または有害作用について報告している臨床研究のシステマティックレビューおよびメタアナリシスを実施した。固定効果モデルとランダム効果モデルを検討した。非ランダム化試験を除外し、感度分析を行った。
結果
システマティックレビューにより、患者 570 例を対象として手術部位感染症を評価した 3 件の研究、および患者 1,082 例を対象として皮膚培養陽性を評価した 4 件の研究を特定した。消毒薬擦り込み＋手術部位スクラブと消毒薬擦り込み単独との比較で、手術部位感染症または皮膚培養陽性に関する有意差は認められなかった。
結論
結論を得るにはさらなる研究が必要である。本メタアナリシスにおいては、1 件の研究のみで有害作用の特定を行っていたが、イベント発生数が少なかったため手法間の比較はできなかった。皮膚が肉眼的に清潔である場合や患者が術前にシャワーを浴びている場合は、スクラブを行う必要はないと考えられる。
