背景
抗クロストリジウム・ディフィシル(Clostridium difficile)効果をうたう消毒薬は、患者への環境由来の感染源となる芽胞に対する効果を確認するための評価を受ける必要がある。残念ながら、現時点ではこれらの消毒薬の評価方法として広く認められているものはない。検査室ではそのような方法がない状況で、医療環境で使用する製品のために作成されたわけではないプロトコールを準用せざるを得ないため、検査に用いる微生物、曝露時間、および合格基準が不適切となる可能性がある。
目的
臨床使用に適した曝露時間および合格基準を用いて、C. difficile 芽胞に対する消毒薬の効果を検査する方法を開発・評価すること。
方法
Healthcare Infection Society(HIS)およびイングランド保健省の Antimicrobial Resistance and Healthcare Associated Infections(ARHAI)諮問委員会からなる合同作業部会を招集した。当作業部会は既報にみられる酵素を用いる芽胞精製法(Clospore 法)を改変し、5 分間の曝露時間で 5 log10 の芽胞数減少という合格基準を採用した。
結果
本法の評価を 3 か所の検査室で実施し、本法は実施が容易であること、および結果には再現性があることが確認された。
結論
作業部会が提示した方法により、純度が高い高濃度の芽胞懸濁液が得られた。環境に使用する消毒薬としては、検査の要件である清潔または汚染条件下で 5 分間に 5 log10 の芽胞数減少を示す製剤が推奨される。

監訳者コメント
C. difficile に対する消毒薬の効果検定で重要なのは、芽胞型菌への消毒効果である。本論文は菌液調整に標準的な方法を規定して安定的な検査を可能にしている点で参考になる。
