本稿では、隔離予防策の終結を目的とした、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)またはバンコマイシン耐性腸球菌(VRE)の保菌が確認されている長期入院中の患者からのそれらの菌の消失を判定するための積極的サーベイランス方針の効果を報告する。30 日を超えて入院中の保菌患者 365 例からの中央値 23 日後の MRSA 消失率は 11%、中央値 26.5 日後の VRE 消失率は 18%であり、これによって 1 年間にわたって 2,152 患者日の接触予防策を節減することができた。この方針は費用対効果が高いことが示された。

監訳者コメント:
MRSA や VREの保菌者に対して接触予防策(CP)を実施しているが、CP による患者への弊害もある。したがって、できる限り早期に CP を解除したいが、「抗菌薬終了して 1 週間経過した後、最低 7 日間空けた 3 回の検査において連続 3 回陰性を確認後解除する」のが一般的である。本論文では自然に排菌がなくなる時期を積極的サーベイランスにより検討し、隔離解除の時期を決めることで費用対効果があるとしている。しかし、保菌者の自然除菌率が 10%~20%と低く、そのうちの再保菌率が MRSA で 20%、VRE で 7%ある。「検出されない≠菌の消失」であり、検査の検出感度以下であることも理解しておかねばならない。ちなみに他の報告では、MRSA 25%~60%(8.5 か月)、VRE 54%(4.7 週)であり、これらを考慮すると、解除のための積極的サーベイランスの是非についてはまだまだ議論の余地があり、データの集積が必要である。
