Clostridium difficile ribotype 027 is most prevalent among inpatients admitted from long-term care facilities

クロストリジウム・ディフィシル（Clostridium difficile）感染症を有する入院成人患者 196 例を対象として、長期ケア施設（LTCF）でのリボタイプ 027 の影響を明らかにするために便中ラクトフェリンおよびリボタイプを用いて腸管の炎症の評価を行った。LTCF からの入院患者（28 例）は、自宅からの入院患者と比較して抗菌薬使用量が多く（P ＝ 0.049）、リボタイプ 027 感染が多くみられた（オッズ比［OR］4.87、95％信頼区間［CI］2.02 ～ 11.74、P ＜ 0.01）。リボタイプ 027 株感染患者は、非 027 株による感染患者と比較して 6 か月死亡率が高く（OR 1.90、95％CI 1.08 ～ 3.34、P ＝ 0.03）、腸管の炎症レベルが高かった（95.26 対 36.08 μg/mL、P ＝ 0.006）。本研究は感染を受けた場所を特定するためにデザインしたものではなかったが、本研究の対象集団からは、患者が入院する前にいた場所は、リボタイプ 027 および重度の C. difficile 疾患と強く関連することが示唆される。

切断酵素分析によって BI、パルスフィールド・ゲル電気泳動解析によってNAP1（North American PFGE Type 1）、そして PCR によるリボタイピングによって 027 に分類されるC. difficile（BI/NAP1/027）が、欧米を中心に流行している。この株はフルオロキノロン耐性を示し、患者の死亡率も高いことが報告されている。本論文は、リボタイプ 027 の C. difficile が長期ケア施設からの転院患者に多く、またその予後も不良であることを示したものである。幸い日本では今のところ、このような C. difficile はまれとされている。
