Outbreak of Clostridium difficile ribotype 027 in a residential home

本稿では、英国の住居型介護施設で発生したクロストリジウム・ディフィシル（Clostridium difficile）リボタイプ 027 感染の重大なアウトブレイクについて報告する。感染した入居者 6 例中 5 例が、診断から 1 か月以内に死亡した。発生施設の調査により、手指衛生および環境清掃に問題があることが明らかになった。感染した入居者は、診断前の 2 か月間に平均 2.7 コースの抗菌薬療法を受けていた。住居型介護施設では C. difficile アウトブレイクが発生する可能性があることを認識することが重要である。ケアを受けている人々の安全を確保するために、医療ケアおよび社会的ケアの両システムが緊密に協働する必要がある。

住居型介護施設における毒性の高いリボタイプ 027 の C. difficile によるアウトブレイクの報告である。6 名の発症者のうち 5 名が死亡（うち C. difficile が直接の死因となった患者は 2 名）であった。手指衛生と環境整備に問題があったとされているが、それに加えて、これらの患者が過去 2 か月間に平均 2.7 コースの抗菌化学療法を受けていた。本報告は、住居型介護施設でもこのようなアウトブレイクが起き得るということよりも、C. difficile 感染症のコントロールには抗菌薬の適正使用が重要であるということが、重要なメッセージだと考える。
