基質特異性拡張型 β-ラクタマーゼ（ESBL）産生大腸菌（Escherichia coli）感染症の発生率が上昇していることから、そのリスク因子を特定して地域の臨床実践に対する情報と指針を提供することを目的として、参加者数をマッチさせた前向き症例対照研究を実施した。症例 54 例および対照 58 例の合計 112 例の参加者を登録した。単変量解析により、過去 12 か月間の ESBL 産生大腸菌分離、および糖尿病の併存が、転帰と有意に関連することが示された。本研究から、臨床実践に有用な疫学的情報を提供するためには service evaluation^※研究が有用であることが示された。

尿路感染に限定した ESBL 産生大腸菌感染症のリスク因子を検討した論文である。自施設の耐性菌対策を評価するとき、対象を限定することはリスクとそれに対する対策を検討する上で有用であろう。
